Player Participation Forms

In order to participate in athletics at Lehman High School, the student-athlete must first complete our Player Participation Forms with the help of their parents or guardians and a physician. The first document that should be completed is the physical done by the athletes physician. The next item is the RankOne forms that can be done online.
